Community Health Assessment and Improvement Plan

Community Health Assessment (CHA)

Assessing the health status of the community is a core public health function of the Green River District Health Department (GRDHD). The Community Health Assessment (CHA) is conducted every three years to collect, analyze, and interpret data about the health and well-being of residents across Daviess, Hancock, Henderson, McLean, Ohio, Union, and Webster Counties.

The CHA seeks to understand what is unique about each community’s strengths, needs, and health challenges. The information gathered helps educate health department staff, local officials, and community members about current health conditions and priorities. It also guides policy development, resource allocation, and community planning efforts to improve health outcomes across the district.

Community Health Improvement Plan (CHIP)

After learning about the health status and health priorities of the Green River District population, GRDHD develops a Community Health Improvement Plan (CHIP). Community health improvement planning is a long-term, systematic effort to address health problems based on the result of Community Health Assessment (CHA) activities, local public health system assessment, and the community health improvement process.

The CHIP is used by the health department in collaboration with other government, educational, human resources and community partners to set priorities, coordinate action plans, and target resources.

How the CHA and CHIP Work Together

The Community Health Assessment (CHA) and the Community Health Improvement Plan (CHIP) are essential tools for understanding and addressing the health needs of the Green River District. While the CHA provides data-driven insights into key health trends, disparities, and challenges, the CHIP turns these findings into action by developing targeted strategies to improve public health outcomes. Together, they serve as a roadmap for creating a healthier community.
