Community Access Project

CommunityAccessProjectLogo_V3Mission Statement

The purpose of the Community Access Project (CAP) is to improve access to healthcare for the uninsured and underinsured of our community and beyond by addressing the physician, pharmaceutical, transportation, case management and inpatient needs of this population.

Green River District Health Department mission is to improve the quality of life by promoting, protecting, and enhancing the health and well-being of the public. In addition, we have a strong mission to develop and maintain healthy habits and lifestyles within our communities. CAP echoes the health department mission by offering services geared towards improving the health status of our community’s uninsured and underinsured population.

Community Access Project in Daviess County was founded in the fall of 2004. CAP is a community-wide initiative created to eliminate healthcare barriers, to “at risk” populations. Patients are assigned donated medical services and prescriptions from a net work of donating medical providers/companies. By managing a physician network, providing prescription assistance, arranging agreements for ancillary and hospital services, CAP will provide enrollees with access to the entire continuum-of-care through donated services from local healthcare professionals and facilities.
