Green River District Health Department (GRDHD) is pleased to present the 2023 Green River District Community Health Assessment. Green River District Health Department (GRDHD) is pleased to present the 2021 Update to the 2018-2021 Community Health Improvement Plan. A Community Health Improvement Plan (CHIP) is a set of goals and actions to address key problems identified through a community health assessment. The 2021 Update of the GRDHD CHIP reports the progress made by local health coalitions and other organizations to improve the health of Daviess, Hancock, Henderson, McLean, Ohio, Union and Webster communities within the Green River District.
